Description 
In ibdm_handle_hca_detach() function, ibdm.ibdm_hca_list_tail pointer is not updated when handling IBT_HCA_DETACH_EVENT and this could potentialy panic the system on a multple HCA system when HCA's are DR'ed out and DR'ed in.
